TO REGISTER AS A CLIENT OF THE LACEY FOOD BANK
All potential clients must register in order to
participate in the food bank program. Registration is held at the food bank Tuesdays & Wednesdays from 10-12 noon.

- Fri from 10-12noon to make an appointment for someone to meet with you and process your application.
Bring with you:
Proof of address: - a recent bill or dated
mail addressed to you and a current driver’s license showing that you are a resident of Lacy.
Income verification: as per current
2014 income guidelines (download below) Submit copies of your IRS 1040, or the last four consecutive pay
checks, unemployment or TANF statements, Medicaid, PAAD, WIC or Food Stamp card).
You must also bring a copy of your children’s
report card ( if in school) or a copy of their birth certificates if under the age of five.
